PATIENT EVALUATION

A.     Subjective:

•    Patient complaint

•    Location

•    Onset and duration of symptoms

•    Description of symptoms (i.e.. numbness. pain [constant, dull, sharp], hypersensitivity. stiffness): use pain scale

Pattern of referred pain

•    Pattern of when symptoms occur (AM. PM. after activity, etc.)

•    Self-help or formal treatment

•    Other medical problems, history

•    Concurrent medications and allergies

B.     Objective:

1.  Inspection:

•    Redness, swelling, warmth

•    Scars

•    Posture alignment

•    Arches

•    Gait Pattern

•    Pelvis and sacrum alignment

2. Palpation:

•    Locate point tenderness

•    Differentiate spasm

•    Differentiate soft tissue tenderness between muscle, ligament, tendon and bone

3. Mobility:

•  Active and passive ROM in all hip. leg and
pelvis muscles

4. Muscle Testing:

•    Tensor faciae latae. quadriceps, hamstrings, hip abductors and adductors, psoas

•    Abdominals, back extensors

5. Provocation Test:

•    Noble compression test for iliotibial-band friction syndrome to rule out disc involvement

•    Straight leg test, test reflexes, ask about disruption in bowel or bladder function

6. Clearing exam for other areas:

•  Test back. hips, legs and feet for secondary
involvement

7. Review x-ray films and physician's report

•    Iontophoresis (medications, dosages, electrode size)

•    Other modalities (e.g., ice or heat, stretching, friction massage, exercise, work hardening, home program, activity or sport re-education to prevent re-injury, etc.)

•    Oral or injectable medications (administered by physician, e.g., NSAIDs, corticosteroids)

•    Pain free exercise when increasing loads may be safely applied
